Am I eligible to apply for a D-SNP plan?
To apply for a D-SNP plan, you must qualify for both Medicare and Medicaid. You need to meet the federal government’s requirements for Medicare eligibility, and your state’s specific requirements for Medicaid eligibility. To qualify for Medicare, you likely need to be:
- Age 65 or older, or;
- Under 65 with a qualifying disability, or;
- Diagnosed with end-stage renal disease (ESRD) or amyotrophic lateral sclerosis (ALS, also called Lou Gehrig's diesease).
If you are dual-eligible, you do not need to wait until the Medicare Annual Enrollment Period (AEP) to choose a plan.
If you qualify for Medicaid and Medicare you may be eligible for more healthcare benefits.
