Health Maintenance Organization (HMO) plans from Wellcare

Our Medicare Advantage HMO plans give you access to our wide network of doctors and hospitals, alongside more benefits than Original Medicare. With an HMO plan you can enjoy cost-savings by receiving most of your care and services from providers in our plan network. While you may have options to go out-of-network for some services, you will enjoy savings by receiving care from an in-network provider. You should expect to choose a Primary Care Provider, or PCP. Your PCP will likely need to refer you to see a specialist.

What does HMO mean?

Health Maintenance Organization (HMO) plans provide coordinated care through a network of providers. Care is provided through a primary care physician (PCP), who may refer people to specialists as needed.

Do I need to pick a Primary Care Physician (PCP)?

With an HMO plan, you need to pick a PCP.

Do I need a referral to see a specialist?

With an HMO plan, you need a referral to see a specialist.

What if I want to see a doctor that is out-of-network?

You can choose to see a doctor that is out-of-network, but you will need a referral from your PCP.

Can I choose a separate Part D plan with my Medicare Advantage HMO plan?

You cannot choose a separate Part D plan with a Medicare Advantage HMO plan, since pharmacies are part of the HMO network.

What benefits do I get with a Wellcare Medicare Advantage HMO plan?
