Health Maintenance Organization (HMO) plans provide coordinated care through a network of providers. Care is provided through a Primary Care Provider (PCP), who may refer people to specialists as needed.
With an HMO plan, you need to pick a PCP.
With an HMO plan, you need a referral to see a specialist.
You can choose to see a doctor that is out-of-network, but you will need a referral from your PCP.
You cannot choose a separate Part D plan with a Medicare Advantage HMO plan, since pharmacies are part of the HMO network.
