Wellcare Dual Special Needs Plans* (D-SNP) are a special type of Medicare Advantage plan designed to support individuals that qualify for Medicaid coverage too. These plans offer all the coverage of original Medicare plus extra benefits for qualifying members at no cost. You will get extra benefits like money for over-the-counter healthcare items (vitamins, toothpaste and more), meal delivery, rides to your doctor appointments and pharmacy, and so much more.

You may also get access to a care manager who can coach you on ways to reach your health goals. As a D-SNP plan member you may be eligible for our EXTRA HELP program which provides additional aid with prescription drug costs.

What does D-SNP mean?

Our Dual-Eligible Special Needs plans (D-SNPs) combine Medicare and Medicaid benefits to offer extra benefits for qualifying members at no extra cost.

Do I need to pick a Primary Care Physician (PCP)?

With a D-SNP plan, you need to pick a PCP.

Do I need a referral to see a specialist?

With a D-SNP plan, you need a referral to see a specialist out-of-network.

What if I want to see a doctor that is out-of-network?

You can choose to see a doctor that is out-of-network, but you will likely pay more for your care.

Can I choose a separate Part D plan with my Medicare Advantage D-SNP plan?

You cannot choose a separate Part D plan with a Medicare Advantage D-SNP plan, since pharmacies are part of the network.

What benefits do I get with a Wellcare Medicare Advantage D-SNP plan?

With a Wellcare D-SNP plan, you can select a primary care provider who will get to know your health needs and coordinate your care with other providers.
