About our C-SNP Plans

Our Chronic Condition Special Needs (C-SNP) plans take a proactive approach to help manage chronic conditions like diabetes, cardiovascular disease or congestive heart failure. WellCare C-SNP plans are designed to provide cost savings on key medical necessities, like insulin, alongside coordinated care to help manage and reduce potential health risk associated with these conditions.

Plan members and caregivers can count on WellCare to support them through their healthcare journey with extra benefits like $0 or low premiums, $0 Specialist Co-pays, and prescription drug coverage.

What does C-SNP mean?

Our Chronic Special Needs Plans (C-SNPs) offer coverage to help members with certain long-term health issues such as diabetes, chronic heart failure and cardiovascular disorder.

Do I need to pick a Primary Care Physician (PCP)?

With a C-SNP plan, you need to pick a PCP.

Do I need a referral to see a specialist?

With a C-SNP plan, you need a referral to see a specialist out-of-network.

What if I want to see a doctor that is out-of-network?

You can choose to see a doctor that is out-of-network, but you will likely pay more for your care.

Can I choose a separate Part D plan with my Medicare Advantage C-SNP Plan?

You cannot choose a separate Part D plan with a Medicare Advantage HMO plan, since pharmacies are part of the network.

What benefits do I get with a WellCare Medicare Advantage C-SNP Plan?

With a WellCare C-SNP Plan, you can select a primary care provider who will get to know your health needs, coordinate your care with other providers and develop a personal care plan to help you manage your condition. You may enroll in the plan only during specific times of the year unless you qualify for a Special Election Period (SEP) or Initial Coverage Election Period (ICEP). Our plans use a formulary. Notice: TennCare is not responsible for payment for these benefits, except for appropriate cost sharing amounts. TennCare is not responsible for guaranteeing the availability or quality of these benefits. Free gift without obligation to enroll. By returning this card, you authorize a plan associate/contracted agent to call or contact you now or during the next enrollment when new benefits information is available. You have the choice to sign up for automated mail service delivery. You can get prescription drugs shipped to your home through our network mail service delivery program.
