What does C-SNP mean?
Our Chronic Special Needs Plans (C-SNPs) offer coverage to help members with certain long-term health issues such as diabetes, chronic heart failure and cardiovascular disorder.
Do I need to pick a Primary Care Physician (PCP)?
With a C-SNP plan, you need to pick a PCP.
Do I need a referral to see a specialist?
With a C-SNP plan, you need a referral to see a specialist out-of-network.
What if I want to see a doctor that is out-of-network?
You can choose to see a doctor that is out-of-network, but you will likely pay more for your care.
Can I choose a separate Part D plan with my Medicare Advantage C-SNP Plan?
You cannot choose a separate Part D plan with a Medicare Advantage HMO plan, since pharmacies are part of the network.
What benefits do I get with a WellCare Medicare Advantage C-SNP Plan?
With a WellCare C-SNP Plan, you can select a primary care provider who will get to know your health needs, coordinate your care with other providers and develop a personal care plan to help you manage your condition.
